Dementia patients can become very agitated and stressed when they feel the world changing around them and become insecure in unfamiliar surroundings. Once a diagnosis of dementia has been made, we will look to see them in familiar surroundings, when appropriate.
Those who have a long term condition are offered care plans, which is used to decide future health intervention. E.g. someone who knows they are dying from cancer may make their wishes known to die at home and not in a hospital. A patient with Diabetes may have advice on how to spot complications arising and can notify clinicians early if they have any symptoms.
